It looks like an enchanting book with no enchantments on it... Try using it in an anvil without actually using it? (Put it plus a tool into an anvil, then mouse over the end product without taking it out).Ehhh I'll post it when I get home.

From the pixelation, and from that the red is pure red, I would hazard a guess that you used Paint to hand-draw text above it, but then realized that your Paint handwriting wasn't good enough to display to us, and so erased it with the eraser. However, you could not figure out how to use the Crop tool to remove that portion.
My EXIF reader is somehow broken, so I can't verify, however, that would be my guess.
Edit: The red dots are anti-aliased, and appear blurry, but their pixel size is smaller than that of Minecraft, which is positive evidence that they have been added in another program. Since you probably wouldn't have used red in Photoshop, as it is not a default color and you probably wouldn't have picked pure red to write with, we can assume that the program used is Paint.
